Your first action

Do this now

Choose the correct program — SSI (need-based) or SSDI (work-record-based) or both — then apply or schedule an appointment with SSA as early as possible. The filing date may matter for back pay.

Next: Social Security Administration general contact

Ask: Ask whether this is an initial claim or an appeal, whether SSI or SSDI applies, and what documents to gather before the interview. SSA sets the requirements.

Not in Louisville? Outside Louisville: civil legal aid for your county

Legal Aid Society serves the Louisville area. Kentucky Court of Justice's Legal Self-Help page links civil legal-aid resources statewide. Social Security's own phone line and field offices take applications and appeals from every county.

Situation: Starting an SSI or SSDI claim, or responding to a denial or appeal. SSI and SSDI have different rules; the exact program must be identified before applying.

What you need to startUse what you have. Missing items do not block the first call.

Have ready if available

  • Social Security number
  • Birth certificate or proof of age
  • Medical records and treating provider contacts if available
  • Work history if applying for SSDI

Missing something? Ask what can be reviewed; do not wait to call.

If the first route fails

If a claim was denied or an appeal deadline has been received: Contact Legal Aid Society for help with a disability appeal before the deadline stated on the notice. The deadline controls — do not let it pass while researching.

Keep any existing SSA application or case number active and continue gathering medical records.

Last checked 2026-08-05 · Kentucky · official source: Legal Aid Society of Louisville — Intake

What remains uncertain: Eligibility, benefit amount, processing time, and appeal deadlines are SSA's determinations. Legal Aid capacity is limited; contact them early.

If the first route is closed or you cannot use it

A closed office, unanswered call, or unavailable device does not prove that you are denied or that a service is unavailable. This guide lists some options, not every possible route. If it is safe to do so, ask for another appropriate contact and keep the original task open.

The office or line is closed

Record what failed and when. Use a listed same-day alternative if one exists, and keep the original call or application as a separate follow-up.

See things that may help today

You cannot use the phone or website

Check for another published channel or ask a trusted helper to open the route with you. Share only the minimum information needed. If no workable channel is listed, that gap remains unknown.

Check the official routes

Related resources can offer another path, but a listing does not confirm current hours, capacity, eligibility, or acceptance. Review related resources.

What this guide cannot promise

Reviewed guidance only. Fees, timelines, eligibility, document substitutes, capacity, and provider practices are not published unless supported by a named source. Confirm current requirements directly.

How to read checked, listed, and unknown informationA listing can stay useful without being provider confirmed.
Checked fact or route
No One Left Outside matched the fact to the named source on the date shown. This is not proof of current availability or provider confirmation.

A listing that may still help remains visible with an honest status such as Possibly active or Needs verification.
PROVIDER CONFIRMED
Only named fields backed by linked first-party provider evidence, integrity checks, and attributed human approval may carry this state. No One Left Outside never infers it from a website, import, likely fit, or contact attempt.
UNKNOWN / Needs verification
No One Left Outside does not know the answer yet. Confirm it directly; this is not a claim that the resource or option does not exist.

Each step in full

The same steps as the plan above, with the instructions for each one.

  1. Step 1Work out which task this is of 7

Private plan · nothing is sent

    Do this

Name whether this is a first application, a records request, a review, or a decision you want to challenge.

    • A first application starts a claim. A review continues one. A denial starts a clock.
    • SSA publishes four appeal opportunities — reconsideration, a hearing before an administrative law judge, Appeals Council review, and a federal district court action.
    • Say which one you are in before you gather anything, because the four ask for different things.

Move on when: you can name which of the four you are in and which office is waiting on you.

Ways to get help for this step

    • Apply for SSI or SSDI with Social Security — national

A first application is filed with Social Security, online or at a local office.

    • Social Security appeals — national

SSA's four appeal levels, if this is a decision you want to challenge.

    Limits and what to confirm

    • Which appeal stage your own notice puts you at. Read it off the notice; No One Left Outside cannot tell from here.
    Why this step is here

First, because a denial and a first application look similar from the inside and are completely different processes with completely different deadlines.

  2. Step 2Read the date on your notice of 7

Private plan · nothing is sent

    Do this

Read the notice and its appeal instructions promptly. Confirm the deadline using the receipt rules that apply to your decision.

    • SSA publishes 60 days to submit a request for reconsideration once you receive a decision, on form SSA-561-U2, and 60 days to request a hearing once you get a reconsideration decision, on form HA-501.
    • Keep the notice and record when you received it. Follow its appeal instructions and ask SSA or a qualified representative to confirm the deadline; do not substitute the printed notice date for receipt.
    • If a deadline has already passed, ask SSA in writing to accept a late request and say why it was late. Whether SSA does is its decision.
    • SSA publishes support on 1-800-772-1213, Monday to Friday 8 a.m. to 7 p.m., with TTY on 1-800-325-0778.

Move on when: the deadline is written down somewhere you will see it, or you have asked SSA in writing to accept a late request.

    Limits and what to confirm

    • Whether SSA will accept a late request, and the rules it applies to that decision.
    Why this step is here

Immediately after naming the task, because this is the only step here with a clock on it. Other requests may also have deadlines; do not wait for records or an intake response before protecting appeal rights.

  3. Step 3Start the application of 7

Private plan · nothing is sent

    Do this

Apply as soon as you become disabled, through whichever SSA route fits your situation.

    • SSA says to apply as soon as you become disabled. SSDI carries a five-month waiting period, so payments do not begin before the sixth full month of disability; there is no waiting period where the disability results from ALS and benefits are approved on or after 23 July 2020.
    • SSI is paid for the first full month after the date the claim was filed, or later if eligibility starts later. Filing sooner is what protects the earlier date.
    • The online disability application is open to someone aged 18 or over who is not already receiving benefits on their own record, is unable to work because of a medical condition expected to last at least 12 months or result in death, and has not been denied disability benefits in the last 60 days.
    • If you cannot complete it online, call 1-800-772-1213 between 8 a.m. and 7 p.m. and a representative can make an appointment; TTY is 1-800-325-0778.

Move on when: the claim is filed and you hold a confirmation, or an appointment to file is booked.

    Limits and what to confirm

    • Eligibility, requested medical evidence, and decision time are UNKNOWN here; SSA determines them.
    Why this step is here

Before gathering documents, because the filing date is what the payment months are counted from and a complete file filed late is worth less than an incomplete one filed now.

Private plan · nothing is sent

    Do this

Write a plain list of conditions, treatment sources, medications, work history, education, and daily limits.

    • SSA publishes an Adult Disability Checklist for exactly this, and a Medical Release Form that goes with the application.
    • Name every place that has treated you, with rough dates. Use the checklist to check for missing treatment sources.
    • Describe daily limitations in plain terms — what you can no longer do, and what happens when you try.
    • Do not mail foreign birth records or Department of Homeland Security documents; bring them to an office instead, where they are examined and returned.

Move on when: every treating source is named with a rough date, and the checklist has no blank lines you could have filled.

    Limits and what to confirm

    • Which records SSA will request and which it will accept. It decides that.
    Why this step is here

DDS handles medical disability determinations; SSA handles applications and technical eligibility. Use the contact on your current request.

    • Kentucky's Disability Determination Services makes disability determinations on behalf of the Commissioner of Social Security for Kentucky residents — and states plainly that it does not take applications for disability benefits.
    • Applications are filed at local Social Security offices, or online at SSA.gov.
    • Technical eligibility — work credits for SSDI, income and assets for SSI — is determined by the local Social Security office, not by DDS.
    • On a disability reconsideration, an examiner from a DDS office reviews the request and the original application. In Kentucky that is a different office from the one that took the application.

Move on when: you know which of the two offices owes you your next answer.

Applications and technical eligibility: Social Security.

    • Kentucky Disability Determination Services — statewide (does not take applications)

    Limits and what to confirm

    • Kentucky Protection & Advocacy publishes 502-564-2967, toll-free 1-800-372-2988, and an online request route.
    • Legal Aid Society publishes an online application and 502-584-1254. Ask whether your location and case are within its current service scope.
    • SSA Form SSA-1696 appoints or updates a representative. General help with forms is not the same as formal appointment.
    • Whether either organisation takes your case is its own decision.

Move on when: you have either asked for help and know when you will hear back, or decided to do it yourself.

    Limits and what to confirm

    • Confirm Legal Aid Society's service area and intake for your case. Applying does not create an attorney-client relationship or stop a hearing.
    • Whether either organisation will take your case, and what it would do about it. Both decide that themselves.

Private plan · nothing is sent

    Do this

SSA will write to you. Make sure the letter arrives and that you can prove what you sent.

    • Record a confirmation or reference for every submission, and the date.
    • If you have no fixed address, the Postal Service publishes General Delivery as a temporary way to receive mail — but a customer can be required to present identification, and each piece is held for no more than 30 days.
    • That matters twice over: it may not solve an address problem for someone also missing ID, and a notice with a deadline further out than 30 days can go back to the sender before you collect it.
    • Call the specific Post Office first. Which facility offers General Delivery, and what identification that postmaster accepts, are decided locally.
    • Choose a safe contact method, name who owns the next action, and write down when to check again.
    • Keep a safe copy or reference number for each notice, submission, referral, and instruction so the work can resume without starting over.

Move on when: there is a route by which a dated letter reaches you, and you can say what you sent and when.

    Limits and what to confirm

    • Which Post Office offers General Delivery near you and what identification it accepts.

If something is blocking you (5)What commonly stops people here, and what has worked around it.

If something is blocking you

A denial or other notice may have a deadline.

Things to try

  • Seek prompt official or qualified legal review; do not rely on an undated summary.
  • SSA publishes 60 days to request reconsideration and 60 days to request a hearing after a reconsideration decision — read your own dated notice against that, because the notice controls.

There is no reliable address, so notices and decisions may not arrive.

Things to try

  • Confirm what address and phone SSA holds before chasing anything else, because every stage after a denial is decided on paper.
  • USPS publishes General Delivery for people not permanently located, but holds each piece no more than 30 days — shorter than a 60-day appeal window.

Medical, work, phone, or mailing records are incomplete.

Things to try

  • Create a source list and ask what can be submitted now versus later.

The Social Security office is closed or you cannot get an appointment.

Things to try

  • Apply online instead: Social Security publishes an online application for disability benefits at ssa.gov/applyfordisability.
  • If you cannot get online, dial 211 and ask for help with the next step.

The normal route stalls, gives conflicting instructions, or cannot confirm the next step.

Things to try

  • Record who said what, when, and through which channel.
  • Ask what escalation or alternate channel exists, then keep a separate fallback route open.

The Social Security Administration publishes four appeal stages and a deadline on each of the first two. It states: "You have four opportunities to appeal our decision" — reconsideration, a hearing with an administrative law judge, review by the Appeals Council, and a federal district court action — and that "You may choose an attorney or other qualified person to help you with the appeal." For the first stage it states: "Once you receive a decision, submit a request for a disability or non-medical reconsideration within 60 days," and names the form Request for Reconsideration (SSA-561-U2). For the second: "Once you get a reconsideration decision, submit a request for a disability or non-medical hearing within 60 days," naming Request for Hearing by Administrative Law Judge (HA-501). SSA states that on a disability reconsideration "An examiner from a Disability Determination Services (DDS) office in your state will review your request and original application" — in Kentucky that is the Education and Labor Cabinet's DDS programme, a different office from the one that took the application. It publishes hearings as held "online, in person, or by phone." Support is published on 1-800-772-1213, "Monday through Friday, 8 a.m. to 7 p.m., in English, Spanish, and other languages," with TTY on 1-800-325-0778. Read the deadline off your own notice rather than this page — the dated notice controls. If a deadline has already passed, ask SSA in writing to accept a late request and say why it was late; whether it does is SSA's decision under rules No One Left Outside has not verified.

The Postal Service publishes General Delivery as a way to receive mail without a permanent address. Its FAQ states: "General Delivery is a mail service for those without a permanent address, often used as a temporary mailing address." The Domestic Mail Manual sets the rules. Section 508.6.1 states general delivery "is intended primarily as a temporary means of delivery: For transients and customers not permanently located" and for customers wanting Post Office box service when boxes are unavailable. Section 508.6.3 states a "general delivery customer can be required to present suitable identification before mail is given to the customer," and that "Prior to mailing, customers should contact the destination Post Office to determine the authorized facility or facilities and their applicable ZIP Code(s)." Section 508.6.2 states general delivery is normally available at only one facility per Post Office, that "A customer may use only one such location," and that a postmaster may refuse or restrict it "To a customer who is unable to present suitable identification" or where mail volume or accumulation cannot reasonably be accommodated. Section 508.6.4 states "Each general delivery mailpiece is held for no more than 30 days, although a shorter time period may be requested by the sender." Two consequences worth planning around: identification can be required, so this may not solve an address problem for someone who is also missing ID; and a notice with a deadline further out than 30 days can be returned to the sender before it is collected. Call the specific Post Office first — which facility offers it, and what identification that postmaster accepts, are decided locally.

If something goes wrong

What to do when a step does not work

A closed office, a failed website or a “no” does not end the plan. Guidance written for this guide comes first. Where this guide has none, No One Left Outside’s general guidance is shown and labelled.

Show the seven situations, deadlines, county differences and what No One Left Outside does not know
If a document is missing

From this guide

  • There is little or no medical record to point at
    • List what treatment did happen, where and roughly when, even without documents.
    • Ask whether an examination can be arranged as part of the process.
    • Start or resume care now — a record that begins today is still a record.
  • The date on the notice has already passed
    • Do not treat a passed date as the end of it. Ask SSA in writing to accept a late request and state plainly why it was late — illness, hospitalisation, homelessness, a notice that went to an old address, a representative who did not act.
    • Ask SSA what it needs from you to consider a late request, and get the answer from SSA rather than from anyone else.
    • Ask Legal Aid Society or another qualified representative to look at it anyway; a missed date changes the argument rather than always ending it.
    • Ask separately whether starting a new application is appropriate, and what that would cost in back-dated entitlement — that is a question for someone qualified, not for this page.
  • Medical, work, phone, or mailing records are incomplete.
    • Create a source list and ask what can be submitted now versus later.
  • Missing: Medical and treatment history
    • List providers and dates as accurately as possible and ask how official records should be requested.
    • Ask first whether these are accepted (not confirmed): Ask the official reviewer what records or release forms are acceptable.

    • Apply online instead: Social Security publishes an online application for disability benefits at ssa.gov/applyfordisability.

No One Left Outside general guidance — not specific to this guide and not an official rule

  • Write down or take a picture of the error message and the time.
  • Try again later, or from another browser or device. Public libraries often have computers to use.
  • Ask the agency whether you can do the same step by phone, on paper or in person instead.

If the phone is disconnected or nobody answers

From this guide

  • The Social Security line cannot be reached
    • Record the source, date, contact channel, reference number, and exact response.
    • SSA publishes automated telephone services 24 hours a day and an online application; use whichever channel is open.
    • Set a named owner and next check for each route that stays open.
  • Nobody can say what is happening with the claim
    • Record the source, date, contact channel, reference number, and exact response.
    • Ask each office what it is waiting for rather than asking both how long it will take.
    • Set a named owner and next check for each route that stays open.
  • No legal help has been found and the date is approaching
    • Ask Legal Aid Society whether anything preserves the deadline while intake is pending.
    • Ask Social Security directly what the deadline is and what must be filed to meet it — a procedural question the agency can answer.
    • Keep applying for representation while doing so; the two are not alternatives.
  • Medical, work, phone, or mailing records are incomplete.
    • Create a source list and ask what can be submitted now versus later.

Kentucky 211 can look for another current route — dial 211 (Kentucky 211 answers 24/7). Call 211

If the request is denied

From this guide

  • The request was not taken up, or the issue is outside its priorities
    • Ask which of its published priorities the issue does fall under, and who else it would refer to.
    • Ask Legal Aid Society whether the issue is within its civil legal help.
    • Put the accommodation request to the agency in writing regardless, and keep a copy.
  • A denial or other notice may have a deadline.
    • Seek prompt official or qualified legal review; do not rely on an undated summary.
    • SSA publishes 60 days to request reconsideration and 60 days to request a hearing after a reconsideration decision — read your own dated notice against that, because the notice controls.

If the service is not accessible

No One Left Outside general guidance — not specific to this guide and not an official rule

  • You can ask for help to use the service: an interpreter, large print, a reader, a different way to apply, or someone to come with you.
  • Ask the office what accommodations they offer, and write down who you asked.

Deadlines

  • If you received a notice or letter, the date on that notice controls. Keep it with you.
  • Write down the date you received any decision or request for more information.
  • No One Left Outside does not track individual deadlines and does not send reminders.
  • Steps in this guide that mention a deadline: Read the date on your notice; Keep contact and confirmations stable.

County differences

  • Offices, hours and local help can differ by county. Use the office that serves the county where the person lives.
  • If you are not sure which office serves an address, ask the agency or Kentucky 211.
  • Some routes in this guide are for Louisville or Jefferson County. Other counties may use different offices.

What No One Left Outside does not know

  • Which appeal stage your own notice puts you at. Read it off the notice; No One Left Outside cannot tell from here.
  • Whether SSA will accept a late request, and the rules it applies to that decision.
  • Eligibility, requested medical evidence, and decision time are UNKNOWN here; SSA determines them.
  • Which records SSA will request and which it will accept. It decides that.
  • Which office currently holds your file. Ask; the answer changes as a claim moves.
  • Whether either organisation will take your case, and what it would do about it. Both decide that themselves.
  • Whether an office is open today, or how long the wait is
  • Whether a program has openings right now
  • Whether a particular person qualifies — only the agency or provider decides
Before you travelA final check of documents, opening hours, and the receiving contact.

Avoid a wasted trip

  1. 1.Recheck the location, hours, appointment rules, and current document instructions before leaving.
  2. 2.Keep originals and copies organized, and ask before handing over the only copy you have.
  3. 3.At the counter, explain what is missing and ask what replacement or workaround evidence can be reviewed.
  4. 4.Ask what was received, what is still missing, how status can be checked, and what happens if the normal route fails.
  5. 5.Keep a safe copy, receipt, confirmation, or reference number for anything you submit.

Ask before leaving the counter: “Is there anything else that could prevent this application from being approved today?”

Practitioner and helper guidance

Preparation ideas for the person and anyone helping
  • Track notices, submission confirmations, and authorized-representative status separately.

Different documents establish different procedural facts.

  • Describe functional observations without diagnosing or promising an outcome.

Eligibility decisions belong to the responsible agency.

  • Never send someone to Disability Determination Services to file a claim.

Kentucky's Education and Labor Cabinet publishes that DDS "does not take applications for disability benefits" — those are filed at one of 27 local Social Security offices or online at SSA.gov. DDS makes the medical determination only.

  • When a claim is refused, establish which agency refused it before deciding what to do next.

The Cabinet publishes that DDS does not decide technical eligibility — work credits for SSDI, income and assets for SSI are the local Social Security office's decision. A refusal on work credits is not answered by more medical evidence, and the word 'denied' does not distinguish the two.

  • For an accommodation, guardianship, or disability-rights question, route to Kentucky Protection & Advocacy rather than treating it as a benefits problem.

P&A is the designated protection and advocacy system in Kentucky, publishes ADA and Section 504 and guardianship among its priorities, and states it responds to requests within 3 business days. It is a different system from the one deciding the claim.

  • Read the notice for which stage it ends, then check it against SSA's published 60 days — do not calculate the date for anyone.

SSA publishes 60 days to request reconsideration and 60 days to request a hearing after a reconsideration decision, and names SSA-561-U2 and HA-501. Knowing the standard tells a helper how much room there is; the person's own dated notice is still what controls, and computing a date for somebody is advice No One Left Outside does not give.

  • Treat health coverage as a separate decision from the disability claim, in both directions.

Medicaid is decided by Kentucky through kynect and DCBS; SSI and SSDI are decided by SSA; and the Medicare Savings Program is a further separate Medicaid-administered decision that SSI or SSDI status does not establish. A refusal in one system says nothing about the others, and people routinely stop pursuing coverage after a disability denial.

  • Sort out where notices arrive before chasing the decision itself.

Every stage after a denial is decided on paper. An unreachable claimant loses on silence rather than on merits, and General Delivery holds a piece for no more than 30 days against a 60-day window.

  • Separate documents, client statements, agency instructions, and unconfirmed assumptions in the record.

Clear labels let the next helper reuse what is known without turning an inference into a fact.

  • End each handoff with a named owner, safe contact method, next check date, and fallback.

A referral is easier to resume when responsibility and the failed-route response are visible.

Day of the appointment or visit

At the office

  1. 1.Arrive early when practical, especially if you are walking in.
  2. 2.Ask for a realistic wait estimate.
  3. 3.Ask whether the office uses text messages, callbacks, or another queue-notification system.
  4. 4.Ask whether you can leave briefly and return without losing your place.
  5. 5.Prepare for a potentially long day.
  6. 6.Write down the name or department that gave each instruction, the date, and the next action.

Bring for the wait

  • A charger
  • Water and snacks where permitted
  • Medications you may need during the wait
  • All paperwork together in one folder
  • Something to do while you wait
  • A safe way to take notes and store confirmation or reference numbers

Journey definition needs-review; checked 2026-08-01. Next review due 2026-11-05. Which office takes applications and which decides medical versus technical eligibility is verified against the Kentucky Education and Labor Cabinet, and the Protection & Advocacy and Legal Aid Society intake routes against their own published pages (all checked 2026-08-05). The SSA application route carries moderate confidence because ssa.gov blocks automated fetching. SSA's four appeal stages, its two published 60-day standards, the SSA-561-U2 and HA-501 form names, representative appointment via SSA-1696, and the DDS reconsideration reviewer were read off rendered ssa.gov pages on 2026-08-06 and carry high confidence. Still unverified and deliberately published as questions: SSA's rules for accepting a late request, Kentucky SOAR (the SAMHSA host does not resolve from this environment, so no SOAR route, contact, or coverage claim is published), Louisville benefits-navigation availability and provider practices, and whether any organisation can take a given case. Outside this guide entirely: every eligibility, evidence, and entitlement question, which require qualified review — publishing a deadline standard is not the same as advising anyone what their own deadline is.

Model-assisted research is planning input only, and a person writes and reviews every published block. No One Left Outside operating doctrine and reviewed SOP or manual material inform only jurisdiction-neutral preparation prompts, not agency rules or provider practices. Supported official facts require a named dated source; conditional claims remain questions to confirm; inferred workflow is nonbinding preparation guidance; case-manager recommendations are labeled practice guidance; Kentucky rules and Louisville provider practices remain pending where identified.
