Have ready if available
- The credential or record you need
- Every document you already have
- Names, dates, and details that do not match
- A safe delivery or in-person plan
Missing something? Start with the task and ask the responsible agency what can be reviewed.
If the first route fails
If a Kentucky ID was lost, stolen, damaged, or destroyed and you need a route without making a phone call: Kentucky says an ID card may be replaced online or by mail for eligible customers, or in person at a Driver Licensing Regional Office.

What remains uncertain: The current state checklist distinguishes ordinary replacement from first issuance and changes. The issuing office decides what applies; local fee help and alternatives marked ask-first remain unconfirmed.

If the first route is closed or you cannot use it
A closed office, unanswered call, or unavailable device does not prove that you are denied or that a service is unavailable. This guide lists some options, not every possible route. If it is safe to do so, ask for another appropriate contact and keep the original task open.
The office or line is closed
Record what failed and when. Use a listed same-day alternative if one exists, and keep the original call or application as a separate follow-up.
See things that may help todayYou cannot use the phone or website
Check for another published channel or ask a trusted helper to open the route with you. Share only the minimum information needed. If no workable channel is listed, that gap remains unknown.
Check the official routesRelated resources can offer another path, but a listing does not confirm current hours, capacity, eligibility, or acceptance. Review related resources.
What this guide cannot promise
Kentucky's current page distinguishes an ordinary replacement, a first credential, changes and Homeless ID. The office must confirm the checklist for your exact transaction. Local fee help, acceptance of substitutes and appointment availability remain unconfirmed.
How to read checked, listed, and unknown informationA listing can stay useful without being provider confirmed.
- Checked fact or route
- No One Left Outside matched the fact to the named source on the date shown. This is not proof of current availability or provider confirmation.
- Directory lead
- A listing that may still help remains visible with an honest status such as Possibly active or Needs verification.
- PROVIDER CONFIRMED
- Only named fields backed by linked first-party provider evidence, integrity checks, and attributed human approval may carry this state. No One Left Outside never infers it from a website, import, likely fit, or contact attempt.
- UNKNOWN / Needs verification
- No One Left Outside does not know the answer yet. Confirm it directly; this is not a claim that the resource or option does not exist.
Each step in full
The same steps as the plan above, with the instructions for each one.
Step 1Work out which ID you need of 8
Private plan · nothing is sent
Do this
Name the exact credential and the office that issues it before you spend money or travel.
- Decide whether this is a replacement of a Kentucky ID you already held, a first Kentucky credential, or a credential from another state. Each is a different transaction with a different checklist.
- For an ordinary renewal or replacement with up-to-date information, Kentucky says no documents are needed. Check this first; do not pay for a birth certificate or replacement Social Security card solely because this guide lists them below.
- For a first ID, Kentucky's page lists a birth certificate, Social Security card and proof of residency, with additional requirements for REAL ID. Name, address or REAL ID changes have their own documents; ask the office for the exact checklist.
- Kentucky's ID card page says: "A $15.00 replacement fee applies." (checked 2026-10-19). Eligible customers can use a verified myDrive account online, mail or a Regional Office. If you cannot verify an account or have no internet, ask a Regional Office about the mail or in-person route.
- Ask the Driver Licensing Regional Office for its current transaction-specific checklist, fee, and appointment rules, and do not order extra records on the strength of an unofficial list.
- REAL ID is a separate choice, not an automatic upgrade. Weigh the extra document burden against how likely you are to need federal air travel or entry to a covered federal facility.
Move on when: you can name the credential, the issuing office, and the checklist that office says applies.
Ways to get help for this step
- Kentucky ID card transaction instructions
For a Kentucky ID card, compare the state's replacement, first-card and change instructions. Other jurisdictions and federal credentials need their own issuer.
Limits and what to confirm
- No-cost Homeless ID requires in-person application at a Regional Office and standard identity/SSN proof.
- Appointment availability, acceptance of alternatives, myDrive account access and local fee assistance remain UNKNOWN.
Why this step is here
Confirm the transaction first so you do not order a record the office does not need.

Step 2Get your birth certificate of 8
Private plan · nothing is sent
Do this
Order a birth certificate if the receiving office needs it, while arranging separate evidence of identity for SSA.
- Kentucky Office of Vital Statistics accepts birth-certificate orders through its published mail, in-person, and authorized online or phone routes. The search and certified-copy fee is $10, and it is not refunded when no record is found.
- KRS 213.141 exempts a qualifying homeless individual, or a qualifying homeless youth under 25, from that fee when homelessness is verified by one of the people or organizations the statute lists. Ask Vital Statistics for the current application process; No One Left Outside has not verified a form, delivery route, or certifier beyond the statute.
- If you were born outside Kentucky, the office that holds the record is in that state. Ask which office controls it and what can be reviewed while a replacement is pending.
- Ask the office you are applying to what other identity evidence it will review while the birth certificate is on its way. Nothing on this page establishes that a substitute will be accepted.
Move on when: the certificate is ordered with a recorded order date and delivery plan, or the receiving office has named what it will review instead.
Ways to get help for this step
- Kentucky Office of Vital Statistics

Limits and what to confirm
- The $10 fee covers the official search and is not refunded if no birth record is located.
- VitalChek is the only authorized online/telephone vendor and charges extra fees. Mail is an offline route; confirm requirements when identity, payment or a safe return address is missing.
- KRS 213.141 lists qualifying homelessness verifiers. Ask Vital Statistics for the current exemption process; do not assume an unverified form or certifier is accepted.
- SSA will not accept photocopies or notarized copies; only original documents or certified copies directly from the custodian.
- Your fee-exemption process, delivery time, and acceptance of substitutes must be confirmed with the responsible office.
Why this step is here
Start a required birth-record request early. SSA does not accept a birth certificate as proof of identity; ask SSA which separate identity document it can accept rather than waiting for this certificate alone.

Do this
Find out whether the office needs the physical card or only the number, then act on that answer.
- Ask the receiving office whether it needs the card itself or will review another record of the number. No One Left Outside has not verified what any particular Kentucky transaction accepts.
- The Social Security Administration says replacement cards are free. Depending on your situation the request may start online; otherwise SSA directs you to make an office appointment.
- Local Social Security offices do not print replacement cards, so plan for it to arrive by mail rather than for a same-day counter result.
- Call Social Security at 1-800-772-1213, staffed 8 a.m. to 7 p.m. local time Monday through Friday, if you cannot use the online route.
Move on when: SSA has taken the request, or an appointment and its document list are recorded, or the receiving office has said the number alone is enough.
Ways to get help for this step
- Social Security Administration National Line

- SSA will not accept photocopies or notarized copies; only original documents or certified copies directly from the custodian.
- The receiving office decides whether the number alone suffices. SSA's accepted identity evidence and the route depend on your situation.
Why this step is here
This follows identity evidence because SSA asks for original or certified identity documents. It is placed before the application because the counter may ask for the number.

Step 4Get proof of where you live of 8
Private plan · nothing is sent
Do this
Find out what the office will review as residency evidence, especially if you have no fixed address.
- With no fixed address, Kentucky uses form TC 94-199: a shelter, health care facility, or social service agency currently treating or serving you attests on it that you have no established and fixed nighttime residence. Ask for the form by number.
- The regulation names TC 94-199, August 2024 edition, for covered adult transactions from August 1, 2024; it names TC 94-198 for qualifying homeless youth aged 16 or 17. The public ID page still describes a signed letter, so confirm the current form with the Regional Office.
- Kentucky publishes a no-cost Homeless ID for eligible residents without a fixed or permanent address. You must apply in person and still provide the identity and Social Security documents the state requires. Ask the Regional Office to confirm current eligibility and the current form before traveling.
- If you do have an address, ask the office which documents in your name it will review, and whether any of them must be dated recently.
Move on when: the office has named the residency document or form it will review, and you know how to obtain it.
Ways to get help for this step
- Kentucky Homeless ID and current ID instructions
Use the Homeless ID section, then confirm the form, signing agency and in-person document requirements with a Regional Office.
Limits and what to confirm
- No-cost Homeless ID requires in-person application at a Regional Office and standard identity/SSN proof.
- Attestation on Form TC 94-199 must be signed by an authorized social service, shelter, or healthcare agency representative.
- KYTC's public page describes a signed letter while the regulation names specific forms. Confirm the current form and your eligibility with the Regional Office.
Why this step is here
Residency is the requirement most often discovered at the counter, and the no-fixed-address route needs another organization to sign something. Both take time that is cheaper to spend before the trip.

Where authoritative sources disagree
Residency Proof for Homeless Applicants
- KYTC Public ID Portal: Instructs applicants without a fixed address to bring a signed letter on agency letterhead from a shelter or social services provider.
- Kentucky Administrative Regulations (601 KAR 12:080): Mandates state-standard Form TC 94-199 (August 2024 edition) for homeless adults, and Form TC 94-198 for homeless youth aged 16-17.
Practical consequence: The public page and regulation describe the residency attestation differently. No One Left Outside has not established how the office combines the letter and form requirements for your transaction.
Safer action: Ask the Regional Office which current letter or state form it requires, who can sign it, and what other documents apply before arranging the trip.
Still unknown
- KYTC's public page describes a signed letter while the regulation names specific forms. Confirm the current form and your eligibility with the Regional Office.
Step 5Fix names or dates that do not match of 8
Private plan · nothing is sent
Do this
Write the mismatch down and ask which record has to be corrected first.
- List every record you hold, the name and date of birth printed on each, and which office issued it. Do not alter or combine them yourself.
- Ask which record must be corrected first and what official or qualified-help route controls that correction.
- Keep both source records. Do not order another record that carries the conflicting detail until the controlling correction step is confirmed.
- Continue the recovery tasks the mismatch does not touch while the correction is pending.
Move on when: an office or qualified reviewer has named which record is corrected first, or you have confirmed the details match.
Why this step is here
A mismatch stops the transaction at the counter, and correcting a record can require its own application. It belongs before the trip, not after a refusal.
Step 6Apply for your Kentucky ID of 8
Private plan · nothing is sent
Do this
Use the route Kentucky currently publishes for your situation, with the documents the office named.
- Kentucky says a lost, stolen, damaged, or destroyed ID card may be replaced online or by mail for eligible customers, or in person at a Driver Licensing Regional Office. Kentucky's ID card page says: "A $15.00 replacement fee applies." (checked 2026-10-19).
- Name, address, and REAL ID changes can require additional documents, so a change alongside the replacement is a different transaction. Confirm it separately.
- If the fee cannot be paid today, ask the Regional Office whether the no-cost Homeless ID applies to you and what proof it currently requires. Other local fee assistance is unverified — ask an outreach organization what it actually offers rather than assuming funds or vouchers exist.
- If the office cannot complete the transaction, ask for a written or clearly itemized explanation of what is missing and which office or record controls the next step before making another trip.
Move on when: the credential is ordered or issued, or the office has named in writing exactly what is still missing.

Step 7Set a safe way to be reached of 8

Do this
Decide how each office contacts you, who owns the next action, and when to check back.
- Choose a contact method you can actually reach, and a safe delivery or pickup plan for anything arriving by mail.
- Name who owns the next action on each open record — you, a helper, or the office.
- Write down when to check again on each one, so a silent week is noticed rather than assumed.
- If you need language or disability access, ask each office what interpreter services, accessible facilities, help with forms, or other accommodations it currently offers. Do not assume an alternative to an in-person visit exists.
Move on when: every open record has a contact route, an owner, and a next check date.
Step 8Keep a copy of everything of 8
Private plan · nothing is sent
Do this
Keep a safe copy or reference number for each order, notice, and instruction.
- Record the order date, confirmation number, delivery or pickup plan, and date received for each record you requested.
- Check that the name and other details on an arriving record match before you rely on it at a counter.
- Store the new record safely and update anything that was waiting on it.
- Keep the reference numbers where the work can resume without starting over if a route stalls or gives conflicting instructions.
Move on when: each record is received and checked, or has a named owner, next date, and fallback.

No Social Security card
Things to try
- Social Security card replacement is free. Depending on the situation, the request may start online; otherwise SSA directs the person to make an office appointment. Local offices do not print the replacement card.
- Ask the receiving agency whether another record can be reviewed while the replacement card is pending.
No fixed address
Things to try
- Kentucky publishes a no-cost Homeless ID for eligible residents without a fixed or permanent address. The applicant must apply in person and still provide the identity and Social Security documents the state requires.
- For residency, the state page says a signed letter may come from a homeless shelter, health care facility, or social service agency and must confirm current services and Kentucky residency. Ask the Regional Office to confirm the current form and eligibility before traveling.
Names do not match
Things to try
- Ask which record must be corrected first and what linking evidence can be reviewed.
The fee cannot be paid today
Things to try
- If the person lacks a fixed or permanent address, ask the Regional Office whether the no-cost Homeless ID applies and what current proof it requires.
- If a Kentucky birth certificate is needed, ask Vital Statistics how to use the KRS 213.141 fee exemption for a qualifying homeless individual or qualifying homeless youth under 25.
- Other local fee assistance remains unverified. Ask an outreach organization what it currently offers without assuming funds or vouchers are available.
No transportation or appointments are far out
Things to try
- Check transportation resources and recheck scheduling early in the morning for cancellations.
Under 18 or records are complicated by incarceration
Things to try
- Ask the agency which additional adult, institutional, or legal records must be involved before traveling.
A replacement was requested but the record has not arrived
Things to try
- Use the issuer’s verified contact route with the order confirmation and ask how status, delivery, or a correction can be checked.
The office could not complete the transaction
Things to try
- Ask for a written or clearly itemized explanation of what is missing and which office or record controls the next step.
- Before making another trip, confirm the exact document, correction, appointment, or route the office says is required.
- Share the explanation with a case manager or outreach organization if you want help organizing the return plan.
A disability, accessibility, or language need affects the visit
Things to try
- Contact the office before traveling and ask what interpreter services, accessible facilities, help with forms, or other accommodations it currently offers.
- If an in-person visit is difficult, ask the agency whether an alternative route or accommodation applies to this transaction; do not assume one exists.
The normal route stalls, gives conflicting instructions, or cannot confirm the next step.
Things to try
- Record who said what, when, and through which channel.
- Ask what escalation or alternate channel exists, then keep a separate fallback route open.

No ID and missing birth/Social Security evidence
Identify which originals/certified documents already exist and the actual credential type. If Kentucky-born and qualifying homeless, locate an authorized VS-41 attester and prepare mailed VS-37/requester-ID packet. Ask SSA about allowable identity alternatives if standard photo ID cannot be replaced promptly; birth certificate alone is not identity for SS-5. Arrange KYTC qualifying residency/service attestation and remaining identity evidence before office visit.

If something goes wrong
What to do when a step does not work
A closed office, a failed website or a “no” does not end the plan. Guidance written for this guide comes first. Where this guide has none, No One Left Outside’s general guidance is shown and labelled.
Show the seven situations, deadlines, county differences and what No One Left Outside does not know
If a document is missing
From this guide
- No birth certificate or an out-of-state birth record
- Ask which replacement office controls the record and what can be reviewed while replacement is pending.
- For a Kentucky birth certificate, KRS 213.141 exempts a qualifying homeless individual or a qualifying homeless youth under 25 from the fee when homelessness is verified by one of the people or organizations listed in the statute. Ask Kentucky Vital Statistics for the current application process; this guide does not claim a form, delivery route, or certifier beyond the statute.
- No Social Security card
- Social Security card replacement is free. Depending on the situation, the request may start online; otherwise SSA directs the person to make an office appointment. Local offices do not print the replacement card.
- Ask the receiving agency whether another record can be reviewed while the replacement card is pending.
- Names do not match
- Ask which record must be corrected first and what linking evidence can be reviewed.
- A replacement was requested but the record has not arrived
- Use the issuer’s verified contact route with the order confirmation and ask how status, delivery, or a correction can be checked.
Kentucky 211 can look for another current route — dial 211 (Kentucky 211 answers 24/7). Call 211
If the office is closed
From this guide
- The office could not complete the transaction
- Ask for a written or clearly itemized explanation of what is missing and which office or record controls the next step.
- Before making another trip, confirm the exact document, correction, appointment, or route the office says is required.
- Share the explanation with a case manager or outreach organization if you want help organizing the return plan.

If the website or portal fails
No One Left Outside general guidance — not specific to this guide and not an official rule
- Write down or take a picture of the error message and the time.
- Try again later, or from another browser or device. Public libraries often have computers to use.
- Ask the agency whether you can do the same step by phone, on paper or in person instead.

If the phone is disconnected or nobody answers
From this guide
- The replacement route cannot be completed or confirmed
- Record the source, date, contact channel, reference number, and exact response.
- Separate any name, address, REAL ID, first-time, or jurisdiction issue into its own pending branch.
- Set a named owner and next check for each route that stays open.
- The transaction checklist is unclear
- Record the source, date, contact channel, reference number, and exact response.
- Do not order additional records solely from an unofficial checklist; keep independently confirmed recovery tasks open.
- Set a named owner and next check for each route that stays open.
- The first Kentucky ID checklist or a record request is unresolved
- Record the source, date, contact channel, reference number, and exact response.
- Keep the unresolved item UNKNOWN with an owner and next check. Use the separate issuer/mismatch plan if the request is not this first adult Kentucky personal-ID transaction.
- Set a named owner and next check for each route that stays open.
- This Kentucky birth-record request is blocked or unresolved
- Record the source, date, contact channel, reference number, and exact response.
- Record the obstacle without guessing or ordering again; keep the issuer's next action, safe contact route, owner and check date visible.
- Set a named owner and next check for each route that stays open.

If the request is denied
From this guide
- This SSA replacement request is blocked or the scope does not fit
- Record the source, date, contact channel, reference number, and exact response.
- Keep the obstacle UNKNOWN with an owner and next check. Use the existing unresolved-issuer plan for a child, foreign-born, original or corrected request; keep Kentucky birth-record recovery in its own scoped plan.
- Set a named owner and next check for each route that stays open.
Kentucky 211 can look for another current route — dial 211 (Kentucky 211 answers 24/7). Call 211
If the service is not accessible
From this guide
- No transportation or appointments are far out
- Check transportation resources and recheck scheduling early in the morning for cancellations.
- A disability, accessibility, or language need affects the visit
- Contact the office before traveling and ask what interpreter services, accessible facilities, help with forms, or other accommodations it currently offers.
- If an in-person visit is difficult, ask the agency whether an alternative route or accommodation applies to this transaction; do not assume one exists.
Kentucky 211 can look for another current route — dial 211 (Kentucky 211 answers 24/7). Call 211
If availability is unknown
From this guide
- The transaction checklist is unclear
- Record the source, date, contact channel, reference number, and exact response.
- Do not order additional records solely from an unofficial checklist; keep independently confirmed recovery tasks open.
- Set a named owner and next check for each route that stays open.
- The correction order remains unclear
- Record the source, date, contact channel, reference number, and exact response.
- Preserve both source records and avoid ordering another conflicting record until the controlling step is confirmed.
- Set a named owner and next check for each route that stays open.

Deadlines
- If you received a notice or letter, the date on that notice controls. Keep it with you.
- Write down the date you received any decision or request for more information.
- No One Left Outside does not track individual deadlines and does not send reminders.
County differences
- Offices, hours and local help can differ by county. Use the office that serves the county where the person lives.
- If you are not sure which office serves an address, ask the agency or Kentucky 211.
What No One Left Outside does not know
- Appointment availability, acceptance of alternatives, myDrive account access and local fee assistance remain UNKNOWN.
- Your fee-exemption process, delivery time, and acceptance of substitutes must be confirmed with the responsible office.
- The receiving office decides whether the number alone suffices. SSA's accepted identity evidence and the route depend on your situation.
- KYTC's public page describes a signed letter while the regulation names specific forms. Confirm the current form and your eligibility with the Regional Office.
- Your eligibility, local fee assistance, required documents, and appointment outcome remain UNKNOWN.
- Whether an office is open today, or how long the wait is
- Whether a program has openings right now
- Whether a particular person qualifies — only the agency or provider decides
Before you travelA final check of documents, opening hours, and the receiving contact.
Avoid a wasted trip
- 1.Recheck the location, hours, appointment rules, and current document instructions before leaving.
- 2.Keep originals and copies organized, and ask before handing over the only copy you have.
- 3.At the counter, explain what is missing and ask what replacement or workaround evidence can be reviewed.
- 4.Ask what was received, what is still missing, how status can be checked, and what happens if the normal route fails.
- 5.Keep a safe copy, receipt, confirmation, or reference number for anything you submit.
Ask before leaving the counter: “Is there anything else that could prevent this application from being approved today?”

Practitioner and helper guidance
Preparation ideas for the person and anyone helping
Ask whether the person is working with an outreach organization.
That organization may know about document-recovery or fee-assistance options.
Ask whether the outreach organization has any current fee assistance.
This is a question to ask, not a promise that assistance exists.
Separate the verified no-cost state routes from unverified local fee help.
Kentucky publishes a no-cost Homeless ID for eligible residents, and KRS 213.141 creates a Kentucky birth-certificate fee exemption for qualifying homeless people; local vouchers and funds still require direct confirmation.
Check language and disability access needs before the visit.
Ask the office what interpreter services, accessible facilities, or other accommodations it currently offers instead of assuming a transaction-specific service is available.
Make an appointment whenever possible.
A scheduled time may reduce uncertainty, but local office practices are not assured.
Plan for appointments to book weeks out.
Field experience suggests long lead times can occur; this is not an agency timeline.
Check appointment availability early in the morning.
Cancellations may create openings, but no opening is promised.
Recheck qualifications and document requirements immediately before traveling.
Requirements and local practices can change.
Track each requested record from order through safe receipt.
Order date, confirmation, delivery or pickup plan, mismatch notes, and receipt status let another helper resume without repeating the request.
Separate documents, client statements, agency instructions, and unconfirmed assumptions in the record.
Clear labels let the next helper reuse what is known without turning an inference into a fact.
End each handoff with a named owner, safe contact method, next check date, and fallback.
A referral is easier to resume when responsibility and the failed-route response are visible.

Usable ID now or REAL ID preparation
This comparison does not recommend one option.
Usable state credential
- Documents
- Current document requirements are pending official verification.
- Cost
- Current cost is pending official verification.
- When it may matter
- When holding a usable credential sooner is the immediate goal.
- When it may not
- It may not answer every future federal-use need.
REAL ID credential
- Documents
- Additional document burden is pending official verification.
- Cost
- Any additional cost is pending official verification.
- When it may matter
- When future air travel or entry to covered federal facilities is likely.
- When it may not
- When the immediate need is simply to hold usable identification.
Do not automatically choose REAL ID. Weigh the extra burden against likely federal use and the urgency of getting usable identification sooner.
