Order a birth certificate if the receiving office needs it, while arranging separate evidence of identity for SSA.
- Kentucky Office of Vital Statistics accepts birth-certificate orders through its published mail, in-person, and authorized online or phone routes. The search and certified-copy fee is $10, and it is not refunded when no record is found.
- KRS 213.141 exempts a qualifying homeless individual, or a qualifying homeless youth under 25, from that fee when homelessness is verified by one of the people or organizations the statute lists. Ask Vital Statistics for the current application process; NOLO has not verified a form, delivery route, or certifier beyond the statute.
