Report a change

A dead number, wrong hours, a program that folded, something we’re missing — tell us. No account needed. Every report goes into a review queue where a person checks it against evidence before anything on the site changes.

  1. STEP 1You send a leadYour report stays private while it is reviewed.
  2. STEP 2A person checks itA reviewer compares the specific claim with evidence.
  3. STEP 3Publication stays separateNothing changes publicly just because it was submitted.

What helps a reviewer confirm this

  • A link a reviewer can open — the provider’s own page, a posted notice, a news item. One good link usually settles it.
  • How you know, if there is no link: “I called on Tuesday and the number was disconnected” is evidence a reviewer can act on.
  • Which specific fact is wrong and what it should say instead, rather than “this listing is out of date”.
  • If you work for the organization, say so — it tells a reviewer how much weight to give your report. It does not publish your change automatically.

Please leave out anything about a specific client, and any documents or identifiers — see privacy & safety. A reviewer never needs them to fix a listing.

An access experience helps reviewers spot patterns, but it is not treated as a verified fact about the resource.

This is a role category only. Do not enter your name, employer, or a client’s information.

Safety note: if this is about a confidential location (like a domestic-violence shelter), please don’t type the address here — write “confidential location” and, if you leave an email, we’ll follow up privately.

Do not submit Social Security numbers, medical records, identity documents, detailed legal records, account numbers, or information that identifies a client. See the privacy and safety notice.

For a factual correction, enter only the specific replacement value if you know it—for example, a new phone number. Leave this blank for an access experience.

If there is no link, briefly identify the public notice, phone call, email, or other source a reviewer should confirm. Do not paste private documents or client information.

Totally fine to leave blank — no contact email is required. If you share it, it stays private, is never published, and may be included in a private reviewer notification or used to ask follow-up questions about this report. Submitting an email means you consent to that limited review and follow-up use.
