This guide is for an appeal of a Determination of Eligibility. Read your notice and the DWD appeal page.

  1. Step 1: Identify the decision

Find the Determination of Eligibility you disagree with. This guide covers that first decision. For a different decision, ask DWD which process applies.

  2. Step 2: Write and sign the appeal

Write why you disagree and sign as the claimant. Include your mailing address, phone number, the last four digits of your Social Security number and a copy of the determination.

  4. Step 4: Use a listed filing route and keep proof

Use a filing route on the first-appeal page. Its first-appeal fax is 317-233-6888. Keep proof of the date you sent the appeal.

  5. Step 5: Prepare for the hearing

Read the hearing notice and follow its instructions. DWD says an appeal you filed will be dismissed if you do not attend. Call 1-800-891-6499 with process questions.
