Childhood vaccine series rewards
Up to $230 by 2nd birthday: Influenza $100 ($50 first shot + $50 second), Rotavirus $50, and $10 each for Hep A, Hep B, DTaP, Hib, PCV, MMR, Varicella and Polio. Plus up to $100 for the HPV series ($50 each dose between ages 9-12).

- Complete the Healthy Rewards Attestation Form; a provider attestation is required for the under-2 series. Providers can print the form at passporthealthplan.com/rewards.
